Is your provider group registered for each Kaiser Permanente area/region to which you are requesting access?

Due to Kaiser Permanente's security controls, your provider group needs to complete a Provider Entity Agreement for each Kaiser Permanente region. Any user registration for a region which does not have this agreement in place will be rejected.

Please review the registration requirements for each region below and confirm the Provider Entity Agreement(s) are completed before continuing with your user registration.
